To obtain copies of your medical records, please call (888) 749-7952. Requests are handled by CIOX, a vendor for Medical City Lewisville.
In order to process your request for medical records in a timely manner, please include the following:
- Completed Release of Information Form, signed and dated by the patient
- Picture Identification
- Instructions for Completing Release of Information Form
If the patient is unable to sign; provide Durable Power of Attorney with letter from physician stating the patient is mentally incompetent.
If the patient is competent but unable to come to the hospital, the patient can write a letter releasing the records to an individual. The patient’s signature will be verified by signature in patients chart.
If the patient is deceased; authorization must be signed by surviving spouse listed on death certificate in addition to the medical chart. If no surviving spouse, provide Executor of Estate documentation.
Birth Certificates for Newborns
The birth process is an exciting time for all parents. The Medical Records Department is responsible for the creation and processing of your baby’s Birth Certificate. Paperwork will be provided to you upon admission to complete. The Medical Records Department will complete any unfinished information, obtain signature/signatures, and file the certificate with the State of Texas.
A copy of your newborn’s Birth Certificate can be obtained from the Denton County Clerk’s Office located at 400 N. Valley Parkway, Suite 2094, Lewisville, Texas, 75067. They can be reached at 972-434-4790 Monday – Friday from 8:00am – 4:00pm.
